barcode for 5essential survey

Beginning January 30, 2024, through March 29, 2024, students, parents, and teachers across Illinois will be able to participate in the annual statewide Illinois 5Essentials Survey. The Illinois 5Essential surveys students in grades 4-12.

This opportunity will allow you to share your thoughts on the essential elements of school effectiveness in a confidential survey about the school. Reports will be generated if the 50 percent response-rate thresholds are met. These research-based reports will be sent to schools and districts in May and will also be included in the State School Report Card, released statewide in the fall.

Please visit https://survey.5-essentials.org/Illinois/ and select the appropriate survey to begin.

All responses are entirely confidential. No names or other unique identifiers are connected with individual responses or used in any report. Survey responses will never be connected to the identity of you or your child.

The Illinois 5Essentials Survey is administered online by UChicago Impact at the University of Chicago on behalf of the Illinois State Board of Education.
