1) Sign in to the SKYWARD software using your assigned username and password.

If you need assistance with a SKYWARD login and your child is NOT missing the appropriate health care documentation for grades K, 6, 9 & 12, please call your school or email webadmin@wabash348.com with your student's name, birth date, and school attending. It would be wise to check your SPAM email folder just in case. Please note the offices will be open on July 31 for questions.

SKYWARD LOGIN

If you have access to more than one portal, you may need to click the drop-down icon next to the HOME button and select Family Access from the drop-down menu.

Click the Online Form Tile.

You will see the Online Form screen, you will see Step 1 of the form.

Click START to begin completing the online form.

Complete the fields or update the fields for the steps as necessary.

Click COMPLETE & NEXT near the bottom left of the screen once you are finished entering information.

After completing all Steps, Click COMPLETE & REVIEW at the bottom of the final required step.

When you have reviewed the information entered for each step, check the box at the bottom of the screen to confirm all the above information is correct. (Checking this box is required in order to submit the online form)

Click SUBMIT FORM at the bottom of the Review step.

To review completed online forms, navigate to > Family Access > Online Form and select My Online Forms History under Features. Then, click Review to the left of the form you wish to review. Only forms with a status of Approved or Auto-Approved will be available for review.

New Students to Illinois:
